A Clinical Review on the Cancer of the Colon Colon and Rectum



Abstract
This report is a clinical review and analysis of 93 cases of colorectal cancer that received diagnosis and surgical treatment form Jan. 1984 to Dec. 1990 at the department of general surgery of the Kang Nam Genral Hospital, Public Corpration.
@ES The results are as follows:
@EN 1) The peak age incidence occurred in the 7th decade of life, and male to female ration was 1 : 1. 16.
2) According to the location of tumor, the most common site of colorectal cancer was the anorectal area(51.7%).
3) The most frequent symptoms and signs were abdominal pain, anemia. Palpable mass in the right colon cancer, abdominal pain, weight loss in the left colon cancer, and bloody stool in the rectal cancer.
4) The duration of disease from onset of symptom to admission was within 3 months in 43.0% and over 1 year in 11.8%.
5) The overall radical resectability was 73.1%, the most common surgical procedure was Miles; operation (23.7%), and the next was right hemicolectomy(20.4%).
6) The most common histopathologic type was adenocarcinoma in 90.3%, well differentiated adenocarcinoma was found in 38.7%, moderately differentiated adenocarcima in 21.5%, and poorly differentiated adenocarcinoma in 14.0%.
7) The distribution according to Dukes; classification was as follows: stage A; 3.2%, B; 32.3%, C; 29.0%, D; 35.5%.
8) The positive rate of preoperative CEA according to Dukes; stage was 0.0% in A, 30.4% in B, 50.0% in C, 70.4% in D.
9) The most common distant metastatic site was the liver(11.8%), the second was peritoneal seeding (10.8%), and then the adjacent pelvic organ(7.5%).
10) The postoperative complications developed in 43.0% of total cases and the most frequent complication was wound infection (19.4%).
11) The three-year survival rate is as follow: Dukes' A, 100%; Dukes' B, 75.0%; Dukes' C, 68.6%; Dukes' D, 6.7%. and the overall 3 year survival rate was 46.8%.
